I have a 2003 BMW 5 series with 5.25" drivers from the factory in the front doors in a sealed (factory) box, I believe.
What are my options (if any) of putting a 6 1/2" driver and utilizing the factory location and sealed box while maintaining the sealed character.
I was thinking of surface mounting with an adapter, but didn't know who to maintain the sealed enclosure that exists in the door already.

often by making a ring out of 1/2" to 3/4" mdf you will raise the speaker out of the mounting enough to clear the basket from the stock hole. You need to make sure there is enough clearence between the the door, and door panel however.
Otherwise, break out the saw!!!!
